Healthcare Power of Attorney

This law product includes the following two do-it-yourself legal documents related to healthcare power of attorney for your U.S. state or location:

1. Healthcare Power of Attorney Document – This document allows an individual (the principal) to designate another person (the agent or healthcare proxy) to make medical decisions on their behalf if they become incapacitated or unable to communicate their wishes. This document typically includes the principal’s healthcare preferences and instructions for end-of-life care.

2. Revocation of Healthcare Power of Attorney – This document allows you to formally terminates the authority granted to the agent in the original healthcare power of attorney. The principal may choose to revoke their healthcare power of attorney if they wish to designate a new agent, create a new healthcare power of attorney with different instructions, or simply no longer want an active healthcare power of attorney in place.
